Abstract:

We present the study we did in the Cartuja Health Centre (Granada). This study has been divided in two large parts; in the first one, we show the way in which the social risk population was identified and the problems they have. In the second one we explain the intervention strategies which a team made up of doctors, nurses and social workers have carried out in a chronological order to improve the health and the social status of such population.
